INQUIRY REPORT

An inquiry was conducted vide letter no ________________________ Dated ___________ from the office of Principal, DIMC, DUHS regarding complaint launched by Mrs. Qaiser Jahan regarding her discharge from E.R of DUH without mentioning the reason.

The board comprises of the following:-

FINDINGS:

  • The board thoroughly investigated the incidence and took written statement of                          Dr. Zil-e-Huma, SMO E.R DUH. As per record (Patient File) the patient was properly attended by Dr. Zil-e-Huma. Proper opinion were sought from medicine and cardiology team, both of them advised to get tests done with follow up in the clinic. They did not advise admission.

  • The patient attendant was insisting to admit the patient as they had approval letter for admission while both medicine and cardiology services did not advise admission.  

CONCLUSION:

The inquiry board has reached the following conclusion:- 

  • Patient was properly seen and managed in the E.R. Due to non-issuance of discharge card policy from the E.R, patient was not intimated the details of her condition in writing.

RECOMMENDATIONS:

  • The inquiry board is of the opinion that any patient discharged from E.R should be provided discharge card bearing diagnosis, treatment and further plan.

  • No patient from E.R should be sent to OPD without discharge card. Opinion from other services should be taken in the E.R.

INQUIRY REPORT


An inquiry was conducted vide letter no /01-021/Dir/DUHS/2014 from the office of Director Medical & Healthcare services DUHS to find/inquire into the circumstances under which Echopac Dongle was lost from Radiology on 11th Oct 2013.

The board comprises of the following:-

FINDINGS:

The board thoroughly investigated the incidence and took written statement of the                  (M T )Medical Technologist Shazia in charge of the room where the Dongle was installed. According to her statement she finished her work at 3 PM and left the office. The Dongle was there when she left the room and when next morning she came to the office at 8.30 AM she found the Dongle missing.

CCtV recording was seen by Mr Sarwar and Mr Nazar I/C control room with Sikander and Mr. Abbas Admin Officers, immediately a day after the incidence and did not find any clue regarding the theft.

The room remained opened/unlocked from 3PM onward till 8.PM and meanwhile the sweepers/ housekeeping staff cleaned the room without supervision. Then they lock the room at 8PM and there is only receptionist Anis on duty from 10 PM till 8.30 AM.

On investigation it was revealed that there is no record maintenance of the equipment nor proper handing over / taking of the equipment is being done when shift changes.

There is no SOP for the operation of equipment and there is no in charge of the rooms to share the responsibility of the costly equipment. Statements of the sweepers on that night were not contributory as they do not know anything about Dongle.


CONCLUSION:

The inquiry board has reached the following conclusion:- 

  • MT Shazia did not have any SOP for the safety of the costly equipment of the room she uses.

  • Do not have any SOP for the use of the devise and its safety, nor there is any register for handing over and taking over of the other equipment like probe and device when changing the shift or closing the room.

  • There is no attendant who should be responsible for the cleaning of the room and equipment and their safety.

  • The room remains unattended and unlocked from the period she left till 8.00PM.

RECOMMENDATIONS:

  1. The inquiry board is of the opinion that the theft was the result of the negligence in part of MT Shazia the I/C of the room who did not manage for the safety of the devise and furthermore left the room unlocked. 

  2. MT Shazia the I/C of the room should be warned to be careful in future so that such incidences should be avoided in future.

  3. There should be a proper SOP for the use of the equipment/devices.

  4. There should be one attendant for two rooms who should be made responsible for the safety of equipment in those rooms and cleaning after the working hours and the room should be locked and key deposited with the cash counter. 

  5. A register should be maintained at the cash counter and entry should be made with date /time , name of the person taking the key and his signature; same should be repeated when depositing the key back.

  6. Such devices which are detachable and easily carried and are costly should be deposited at the cash counter after the use and entered in the separate register with date, time and the name and signature of the person.

  7. It is recommended by the inquiry committee to recover the loss from MT Shazia who showed irresponsible attitude towards her duty to manage the equipment’s safety. 50 % of the cost to be beard by MT Shazia and rest 50 % by the University.
