An incident reported to Director
Medical & Healthcare Services on 02 Oct, 2014 that a patient named Bibi
Zahida MR # 17421 fell down from the OT table in OT # 1 in the recovery phase
from General Anesthetist.

Inquiry (Facts)
During the inquiry, proceeding
following facts came into light.
·
Patient was left on the OT table
alone with anesthetist and medical technologist. On duty, Dr. Ghulamullah and
Mr. Osama (OT Technician) were absent.
·
Mr. Osama was asked to go to OT # 2
for quick preparation of Lap Chole Tower so that there may not be any delay in
starting the next surgery & he was ordered to do so by Dr. Tayyab.
·
The belt fastened on the lower
abdomen during Lap chole surgery was not tied properly.
·
So as certain fact, Dr. Tayyab was called
in his statement attached for reference.
Observation
·
In my opinion, it was an accident
& was not a deliberate act, which took place unfortunately.
Conclusion
·
In my opinion, until & unless
patient is shifted to recovery, assistant doctor and OT technician must stay at
the side of the patient
·
There is a shortage of OT staff
which has to be seriously overcome.
·
SOP of OT should be followed.

Due to the forecast of
cyclone (NILOFER) in Sindh, Disaster
emergency is announced in Dow University Hospital Ojha campus from 31th
Oct. till 4th November. It is therefore mandatory for all the staff working in
emergency, administration ,nursing ,O.T, Pharmacy, Laundry and House keeping
department to make them available in hospital during these days, their holidays
will be adjusted later on.
