CASE SUMMARY
40 Years old female patient Mrs. Babai K/C of HTN for 2 years with a complaint of Neck Pain for 12 years & NECK Swelling for 12 years. She was diagnosed as a case of multinodular goiter so subtotal thyroidectomy was done by Dr. Parvez Iqbal in Dow University Hospital. Her operated findings include enlarged Left Lobe Right Lobe contained calcified nodules approximately. She was admitted in Emergency for 1 week after the removal of stiches.

CASE SUMMARY
This is with reference to your letter dated 21-5-2014. A 28 year male patient, Mr. Mir Jan S/O Ali Nawaz, resident of Larkana, K/C of Hepatitis-C, CLD, admitted through emergency on 12-1-2014 with complaints of
Generalized weakness.
Black stool for 1 month.
Vomiting for 4 days.
General.
O/E = Anemia++.
Jaundice +
Odema, Clubbing, Cynosis - negative
Systemic exam = No positive findings
Impression.
Upper Gastro Intestinal bleed secondary to decompensated Chronic Liver Disease.
Hospital Course.
Patient was admitted in male medicine ward, investigations done that show Hb % of 4.7, Patient was shifted to Intensive Care Unit where 3 packed cells were transfused, I/V terlipressin was started.
Patients EGD and Colonoscopy done. Colonoscopy revealed mid gut bleed, cecum and terminal ileem smeared with altered blood.
CT abdomen with contrast is normal with splenic varices.
Patient was advised CT angiogram to see coeliac axis and discharged on Tab. Ferrous Sulphate 200mg TID, Tab. Folic acid 5mg OD, Cap. Esvin 40mg OD and Follow up in OPD with CBC report after 2 weeks.
List of the entire test advised is attached. Reports were given to the patient at the time of discharge.

This is bring into your kind knowledge that as Mr. Raheel was expelled from DUHS, Ojha Campus and currently working under third party which will look after the affairs of oxygen maintenance only and refused to provide Oxygen Operation Services.
Mr. M. Amjad Ali, regular employee is transferred to (O & M) as a supervisor. So we are facing problems in operation services of oxygen supply in Dow University Hospital currently. We have not released Mr. M . Amjad Ali from his previous duty of operation services of oxygen.
So it is requested to either allow him to continue his duty as oxygen operation supplier or provide 24 hours qualified staff, on urgent basis.
